Failure to follow an “air management doctrine” and standard procedures for fighting a high-rise fire likely contributed to the death of an Asheville fire captain last year, a report from a federal agency that investigated the fire concludes. The report from the National Institute for Occupational Safety and Health, released to the public Monday, also suggests that Capt. Jeff Bowen may have shown signs of carbon monoxide poisoning before he ran out of air inside the building at 445 Biltmore Ave.
