Ohio fire chief, others internally charged for missing, erroneous records at fire training academy

Cleveland's outgoing fire chief and two supervisors will face internal charges after an audit revealed dozens of missing and erroneous records at the department's training academy. The charges, outlined in letters released by the city Tuesday, make Patrick Kelly the third consecutive chief to leave the embattled fire department under a cloud of controversy. Kelly recommended the charges against himself, Battalion Chief Sean DeCrane and Capt. Patrick Corrigan after an audit performed by the city's Integrity Control Unit showed that the academy failed to keep proper records of continuing education courses, according to the city's findings. The state requires the department to keep such records. The audit was sparked by a complaint filed by an academy employee who said he was instructed to enter false information into a database that tracks continuing education courses, according to the city. The unit randomly inspected records from 36 continuing education classes the academy held in 2014. While the investigation turned up no evidence of criminal wrongdoing, it revealed that neither instructors nor attendees signed attendance logs. The names of instructors and attendees were missing from dozens of records, and 11 attendance logs were completely missing, according to the city.
