Huntington officials released an external review report Friday of the city's fire department.
According to the report, the review showed that blame for the improper maintenance of the city's fire fighting equipment could not be placed on one individual. Instead, the report describes the situation as "a series of failures" that led to the issues.
"It is important to note that although there have been management relations issues, the daily successes of the department have been tremendous," the preface of the report states.
The West Virginia Department of Military Affairs and Public Safety (DMAPS) conducted the review after being contacted by Mayor Steve Williams about "long-term issues" with the fire department, according to the report.
